Follow below to learn how to log in, install, and set up the Forms for Salesforce app correctly for the first time.
Note: If you've already installed the application and you're launching Forms for Salesforce for the first time, you'll want to follow these steps detailed below for launching the application.

Log in to AppExchange
Step 1: Navigate to the Formstack AppExchange listing or search Forms for Salesforce by Formstack.
Step 2: Click "Get it Now."
Note: If you'd like to test Forms for Salesforce in a Sandbox before installing it in production, you can check out the "Try it for free option".
Step 3: From the pop-up, select "Log in" and enter your production credentials.
Note: A Salesforce org must be attached to your Salesforce Trailblazer account in order to install this app. If you have not created a Salesforce Trailblazer account, use this link to create your production credentials.
Install the app
Once you have logged in, you will be taken back to the Formstack App Listing. Here, select “Get it Now” to install.
Step 1: Choose the Environment. Here, select "Install in Production".
Step 2: From either environment, check the "Terms and Conditions" box.
Step 3: Select "Confirm and Install" to complete.
Setup the Formstack Salesforce App
After you have confirmed and installed the app, you’ll be redirected to a login screen. Log in using the production username and password.
Step 1: Select which profiles to install the app for and click "Install."
You can choose between:
- Admins
- All Users
- Install for Specific Profiles
Note: The Install for specific profiles option lets you be granular with the permissions for each user type.
Step 2: Grant third-party access and select "Continue."
Step 3: Wait for the installation to finish.
Note: If the package states it is taking too long to install, you will receive an email once the installation has finished.
Post Installation Steps(Create or Add an existing Force.com Site)
After the Forms for Salesforce app has been installed, you'll now have the option to open the application from your app launcher on the top left.
After launching this app for the first time, you'll have a few steps that you'll need to complete before jumping into creating your first form. The first step will be to assign who will be the primary user.
After you've assigned the primary user, you'll have a few more options to complete before jumping into creating your first form.
The first mandatory step is to create or attach an existing Force.com site. For more information from Salesforce on creating subdomains and Force.com sites, check out this article. If you don't have an existing site, you can choose No from the options and then you'll see an option to create one as seen below.
When creating a Force.com site for our Forms for Salesforce application, it's important to make sure that you've set the page to active and disabled the cache public visual force pages option; otherwise you’ll potentially run into some issues with prefill.
Here's an example of how this page could look with some of the required option configurations highlighted.
After you've saved your site, you'll be able to select it from the dropdown or copy and paste the URL from the site page.
(Optional)The next option is to get keys for Google ReCAPTCHA
After finishing the previous options, you can click Next to continue and finalize all of your settings. After this, you'll have finished your setup and will be taken to the homepage to create your first form.
