Signing up for an Account

You can sign up for a new account here. You will need to enter your first & last name, email address, and phone number.  Formstack will send you a password and verify your account by sending an activation email to the email address you used when creating your account. If you don't see this email, please check your spam folder.


Plan Options

We offer several different plans: Pro, Teams, and Starter. The difference between the paid plans is the number of forms allowed, the number of submissions you can store in the Formstack database, your number of user accounts, the number of style templates, and the amount of storage space you get for file uploads. Professional Services

Formstack offers Professional Services for those looking for custom, technical, or a more hands-on experience.  Choose between packaged solutions that deliver just what you need or consultative solutions for your specific form building or account needs.


Payment Methods & Invoicing

We accept MasterCard, Visa, and American Express credit card payments. We also offer annual invoices for Platinum plans and higher. The non-profit discount applies to invoices but the annual discount does not. To process your Formstack payment by Invoice on a Platinum plan or higher, please complete and submit this Form.  Please note that certain restrictions will apply if you do not pay via credit card through the standard billing system.


Payment Terms

We offer monthly and annual plan options.  For plans paid annually by credit card, we offer a 2-month discount.  We offer monthly and annual plans through our sign-up page and you can review all pricing and options through our plan comparison page, here.

Refund Policy

If you decide to cancel your account within 30 days of signing up, we'll give you a full refund no questions asked.  Just check the refund box on the cancellation page.  If you're inclined to share why you want to cancel the account, we'd love to know that too.
