Building your Form
To begin, login to your Formstack Forms account and click "Create" from the Home or Forms tab.
Tip: If you'd like to organize your forms by folder, check out this article for more information.
Note: If you do not see this button you may not have permission to create forms. Please contact your account admin so he or she can adjust your permissions settings.
Next, you will have the option to choose the type of Form that you're looking to create. The Form option allows you to collect as much data as possible on one or more pages while the Survey form type is meant for collecting sentiment data and is limited to 1 field per page.
Tip: You can always change the Form type from Form to Survey under the Form extras tab while on the builder, but the option to generate a Form based on a prompt with AI is only available on this step at this time. After you've selected your Form type, you'll have the option to name your form & add a URL. The form URL will automatically populate with the form name. If you've chosen the Form or survey option, you can edit any of our dozens of templates or choose to start with a blank form.
Tip: The URL can be changed at any time after the Form has been created from the Share tab. While the sub-domain can also be updated with instructions from this article on the URL settings. It's important to update any existing links to avoid errors if you do make changes to the URL.
After you've chosen the format of your form, you'll be taken to the form builder. Use Formstack’s drag-and-drop builder to pull fields from the menu on the left onto your form on the right.
Tip: Easily mark fields as required, add sections for organizing your form or configure conditional logic for showing or hiding fields based on conditions.

Customize the look of Your Form with a Theme
You can customize the look and feel of your form by adding your logo and brand colors.
To create your first theme, navigate to the Build tab on the form you’d like to customize. Select Style in the upper left hand corner. You can create a new theme by selecting the Create Theme button or apply an existing theme to your form.
In your new theme, start by giving your theme a name. You'll see Quick Styles at the top, which are easy, quick edits. Open up the Advanced Styles tab to unleash all the theme options you can customize.

Once you’re happy with your Theme, it's time to save! Choose to Save, Save as New, or Save and Activate the theme. Save and Activate will save your new theme and set it as the Active theme on your Form.
Tip: One thing to keep in mind is that Themes only apply to forms that are hosted by Formstack. If you're embedding a form on your own website, it will include the styling you applied to your form via your custom Theme, but will not include styling outside of the form box and may be affected by the styling of the web page where you've embedded. In this case, you may need to alter the CSS on your Theme. You can do this in the Advanced Code Editor inside the Theme Builder.
Form Settings
Click the Settings tab to gain access to a number of different features, including:
-General Settings (Set the form name, choose the language, adjust form status such as submission limits, and toggle options to optimize performance for longer forms).
-Emails & Actions on submissions for setting confirmations emails for end users and notification emails for internal stakeholders.
-Welcome & Submission Messages that would appear on a form.
-Adding an extra layer of encryption through the Security settings page.
-The ability to setup User Access and determine who should be working on the Form.

Sharing your Form
Click the Share tab to find the link to your form (the Hosted URL) and the embed code needed to place the form on your website. The Quick Start menu provides the link to your Form and also the JavaScript Embed Code, which can both be quickly and easily copied by clicking the 'COPY' buttons. You can also access Lightbox and iFrame embed codes by navigating to the Website Embed section.
Managing Submissions
Submissions are captured when a Formstack Form is completed and submitted. The data is stored within the forms database and accessible from the forms submission table to view and manage. For more information on viewing & managing your submissions, check out this article.
