When creating a document you may need to add some additional text for your recipient. This can be completed by assigning text fields to the participant while on the prepare document step as shown below. this can be adjusted for the first or any additional participant to complete during your signing process.
Once this is completed you can add additional recipients as normal and proceed to the Emails & Actions tab once it's been completed. After you've completed this step, you can click send to start the signing process.
The Document will go to whoever was assigned first to complete your signing process as well as add your additional text that was added to that participant.

Once you have completed your process the document will be sent to your recipient to complete and the additional text will be included on your document.
