If you see the message:
“Sorry, we could not locate that form. Please check the URL for accuracy and try again. If you are still having trouble, please reach out to the support team.”
This usually means the form is no longer publicly accessible. There are two common reasons this happens. Follow the steps below to resolve the issue.
1: Check if the hosted form is disabled
Even if a form exists in your account, it will not load if the hosted version has been disabled.
Steps:
- Open the form
- Go to Settings
- Locate the Disable Hosted Form toggle
- Make sure this setting is set to No
- Save your changes if needed
When Disable Hosted Form is set to Yes, anyone visiting the form’s URL will see an error message.
2: Check if the form was deleted
If the hosted form is enabled, the form may have been deleted and moved to the Deleted Forms folder.
Steps:
- In the left-hand menu, click Deleted Forms folder
- Look for the missing form in the list
- Check the box next to the form
- Click Restore at the top of the page
Once restored, the form will return to your active forms list and should be accessible again using its original URL.
